The Clerk of the Circuit Court proudly introduces the next
generation in on-line court forms---forms with fillable fields

  The forms listed below are designed so that users can fill in the blanks using their personal computer word processor. Completed forms can then be printed locally and filed with the Clerk's Office per court rules. At this time, electronic, on-line or e-mail filings are not acceptable under the court rules currently in effect.

INSTRUCTIONS:

1.       You must have Adobe Acrobat free Reader loaded on your computer. It is available for
download and install according to the directions provided at Adobe's web site.

2.       Choose which fillable forms you would like to use. At this time, fillable forms are only available from this page. In the near future, as we add to our fillable forms selection, you will be able to access those forms from the main forms page. At that time, fillable forms will be identified with an asterisk after the form title.

3.         Click on the selected form. Your browser should launch Adobe Acrobat automatically and download the form to your computer. You will then be viewing the form using Adobe Acrobat Reader. You may use Acrobat Reader to view and print the document.

4        Once the form is displayed, click above any of the blank lines where you usually type text. Your cursor will be displayed above that line. Type appropriate text. When you complete the text for that line, hit the <Tab> key and the cursor will automatically move to the next fillable field. Note that the fillable fields are equipped with automatic line-wrap and shrink to fit features. If you type more characters than fit on one line, the text will automatically wrap to the next line. When you press <Tab> to move to the next field, the text that you just typed will automatically shrink to fit the field.

5.       When you have filled all fields, you should print the form. Please note, the technology allows only for filling and printing locally. You cannot save the filled forms. When you have completed filling and printing the document, exit Acrobat Reader to return to your browser. You will not be able to save the completed form to your local computer.
