Master Subscriber Agreement
Agencies must sign a Master Subscriber Agreement with the Clerk of the Cirucit Court to obtain access to information maintained on the CCC Portal System
Request Form for CCC Portal Login Access (.pdf)
Agency Account Coordinators must use this form to submit requests for new user accounts, changes to existing accounts, or the deactivation of user accounts. This request form is intended for the entire agency, not an individual user.

Agency Account Coordinators will use this spreadsheet to log the data access requests for their users to CCC Portal.

Each individual user that is requesting “elevated” access to CCC Portal must complete this form. The Agency Account Coordinator must provide a copy of this form each new user account request.

FAQs

Your username is the e-mail address you entered when you registered for CCC Portal.
Typically, one Agency Account Coordinator is appointed per agency, but the Clerk’s Office recognizes how appointing one AAC could be challenging for a large agency. In this case, you should decide how best to group your users. Provide the names, contact information (including email addresses), and group description for each designated AACs in an email to cmsuseraccess@cookcountycourt.com.

For example, you may decide to break your users up by their assigned district. In this case, you should appoint a designated AAC for each district.
The first task you want to do is to complete and have the Master Agreement signed by your agency head and send it back to us as cmsuseraccess@cookcountycourt.com.

Next have all of the users that will need to access the system watch the different videos listed under the Online Training Webinars and download the training guide.

For each of your users, have them sign the CCC Portal Elevated Access User Agreement, save a copy for your files and scan them because you will need to send them with your request for access.

For each set of requests, i.e., an initial request for new users, you will provide a completed Request Form for CCC Portal Login Access form and an User Management Tool - Data Access for CCCPortal MS Excel spreadsheet. In the spreadsheet you will list all of your users you are making this request for, indicate using the drop-down which additional access they should be GRANTED, and then in the Action column select that these are a new user. When you send a request for modification to existing access or deactivation you will select one of the other options.
